Dear Mr. Arnold,
As per the health and safety policies of our school, this is to inform you that your area would need to register for some activities that would take care of your child. As our school follows to the rules and regulations of the state law of health and safety of children we have taken this plan to keep legal health records of the children too.
The medication and other medical procedures would be performed under your supervision. The legal forms would require your approval and signature .It is based on the student's health conditions and the medical records provided by you, we would like to take the measures accordingly.
In case of emergency in school your ward would be provided with medical help on the basis of his reports. Please make a note of the seriousness of this intimation.
